Urgent - Permission and Medical information Required

URGENT: LATEST PERMISSION AND MEDICALINFORMATION REQUIRED FROM ALL KL ENSEMBLE MEMBERS

As our dates have altered from the original permission form you completed at the end of 2008, we now need an updated permission note completed and returned by Thursday 10th April. As part of the update we also need the Medical Information Form completed again to ensure we have correct information. The form is in the Ensemble documents folder.
